Application for a grant from the Icelandic Physical Therapists Association (FS) Science Fund

Please read the instructions carefully before filling out the application.

Since there is probably not enough space on the form for all the information in sections 11 - 15, additional information should be submitted on a separate sheet, referring to the relevant number on the form. Please also include information about your academic and professional background, along with your publications for the last 5 years, on a separate sheet with the application. If there is more than one applicant, the names of all applicants, along with information corresponding to sections 1 - 8, and information about your academic and professional background, along with your publications for the last 5 years, should also be submitted on a separate sheet with the application. The applicant must submit the following supporting documents at the bottom of the form as required.

The application and supporting documents must be sent by email to the Science Fund Board.
